Job Summary

As a Hospice Care Specialist, you will provide compassionate care to patients and their families, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities
  • Conduct thorough nursing assessments to identify patient needs and develop individualized care plans
  • Provide direct patient care, including medication administration, wound care, and emotional support
  • Collaborate with the healthcare team to coordinate care and ensure seamless transitions
  • Document patient progress and communicate effectively with families and healthcare providers
Requirements
  • Current, active LPN licensure in the state of South Carolina
  • Minimum 1 year of experience in hospice or palliative care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
