We are seeking a skilled Social Worker to join our team at Deerings Nursing & Rehabilitation. As a Social Worker, you will play a vital role in ensuring the well-being and quality of life for our residents.

Key Responsibilities:

  • Develop and Implement Care Plans: Develop comprehensive social histories of residents, and create personalized care plans that address their medically related social and emotional needs.
  • Collaborate with Interdisciplinary Teams: Participate in care plan conferences and contribute to the development of care plans for residents, ensuring a cohesive and effective approach to their care.
  • Provide Support and Education: Offer individual and group support to residents and their families, addressing their needs and concerns during times of adjustment, crisis, or transition.
  • Coordinate Discharge Planning: Work closely with staff, residents, and families to ensure a smooth transition to post-acute care or other settings, as needed.
  • Facilitate Social Services and Education: Coordinate in-service and educational programs for staff, promoting best practices in social work and enhancing their skills and knowledge.
  • Promote Facility Programs: Develop and implement strategies to promote our facility's programs and services, ensuring priority consideration for referrals from healthcare professionals, community agencies, and other stakeholders.

Requirements:

  • A Bachelor's Degree in Social Work (BSW) or a related field, with certification as a social worker designee.
  • Excellent interpersonal and communication skills, with the ability to build strong relationships with residents, families, and staff.
  • Experience working in long-term care settings, with a strong understanding of the complexities and challenges involved.
