Job Title: Direct Support Professional

PermiaCare is seeking a dedicated and compassionate Direct Support Professional to join our team. As a Direct Support Professional, you will play a vital role in supporting individuals with intellectual and developmental disabilities to live their best lives.

Job Summary:

The Direct Support Professional will provide residential, individualized skills and socialization, day activity, crisis, and community supports in compliance with Contract standards. This position requires the ability to work independently, under limited supervision, and to maintain accurate documentation of services.

Responsibilities:
  • Provide personal care assistance, including bathing, hygiene education, mobility, and toileting assistance.
  • Support individuals with daily living activities, such as meal preparation, medication monitoring, and assistance with household chores.
  • Ensure frequent communication and interaction with individuals to maintain a positive environment.
  • Model appropriate behaviors for individuals served.
  • Assist with monitoring, daily living activities, behavior supports, personal care, and ensuring the physical and emotional needs of individuals are met.
  • Provide transportation to outings and ensure individuals' participation in all daily activities, including cooking, cleaning, and social events.
  • Document services accurately and complete documentation in accordance with PermiaCare policies.
  • Provide respite services as needed and assist with loading and unloading individuals into and from vans.
  • Ensure individual rights are respected and protected.
  • Provide companionship and conversation to individuals in care.
Requirements:
  • High School Diploma or GED.
  • At least 1 year experience working with individuals with intellectual and developmental disabilities or related conditions.
  • Must maintain a valid Texas Driver's license, auto liability insurance, and a driving record acceptable to PermiaCare's insurance requirements.
  • Required to pass criminal history and background checks as well as pre-employment drug screen.
  • Nurse Aid certification preferred.
Working Conditions:

This position requires the ability to work in a fast-paced environment, with frequent changes in schedules and priorities. The Direct Support Professional must be able to lift, bend, and move individuals as needed, and to work in a variety of settings, including residential and community-based settings.
