Key Responsibilities:- Develop trusting relationships with patients and families as an end-of-life care expert
- Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient
- Continuously evaluate personal and professional performance to increase productivity and quality of care delivered
- Make initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral
- Initiate and coordinate the plan of care, documenting problems, goals, interventions, and patient/family response to hospice care
- Collaborate with patients, families, attending physicians, and other members of the IDG to provide daily patient and family care
- Instruct and supervise patients and families in self-care techniques when appropriate
