Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our hospice care team. As a RN Case Manager, you will be responsible for providing direct patient care services, collaborating with an interdisciplinary team, and empowering patient caregivers through education and support.
Key Responsibilities- Provide direct patient care services to hospice patients and supportive care to their families
- Act as primary liaison between physician, patient/family, and hospice team
- Manage and collaborate with an interdisciplinary group to provide comfort, peace, and dignity
- Evaluate patient and family needs and assist in the preparation and execution of the plan of care
- Maintain all assigned patient clinical records
- Participate in company quality assessment and performance improvement programs
To be successful in this role, you will need to possess a RN license in the state of employment, 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology, and a strong ability to work in a fast-paced environment. Additionally, you will need to demonstrate flexibility, composure, and a compassionate approach to patient care.
