Position Overview

Key Responsibilities of the MDS Coordinator:

  • Establish a schedule in accordance with regulations for the IDCP team to ensure timely and precise assessments.
  • Complete sections of the MDS form utilizing various sources of information, including resident records, input from residents and their families, direct care staff, and licensed professionals.
  • Certify the completion of assessments by signing and dating the MDS form at R-2B.
  • Participate in weekly Medicare meetings to present patient status, progress, and assist in discharge planning.
  • Attend additional facility meetings as requested by the Administrator and provide reports based on the meeting objectives.
  • Act as a liaison between the rehabilitation department, center staff, physicians, and families, maintaining a courteous and respectful demeanor.
  • Conduct quality assurance activities and report findings to the QA committee, collaborating with facility leaders on performance improvement initiatives.
  • Ensure the timely and accurate completion of MDS and CAA assessments in compliance with regulations.
  • Engage in facility orientation programs as needed.
  • Plan and participate in in-service training related to MDS services and regulatory compliance.
  • Guarantee the timely submission of all MDS assessments to CMS.
  • Maintain a clean, organized, and safe work environment.
  • Stay updated on industry changes by attending workshops and seminars to uphold professional standards.
  • Protect the confidentiality of all patient and resident care information to uphold their rights.
  • Demonstrate the ability to anticipate and respond to customer needs effectively.
  • Adhere to all safety protocols and company policies.

Qualifications:

  • Graduate from an accredited nursing program with MDS experience preferred.
  • Current RN or LPN license in the State of NY.
  • Ability to read, write, and comprehend written instructions.
  • RAC certification is highly encouraged.
