Job Summary:
The MDS Coordinator plays a pivotal role in ensuring the timely and precise completion of assessments and interdisciplinary care plans that comply with Federal and State regulations. This position is essential for coordinating the Resident Assessment Instrument (RAI) process, ensuring that the Minimum Data Set (MDS), Care Area Assessments (CAA's), and interdisciplinary care plans are completed accurately and on time.
Duties/Responsibilities:
- Guarantees that the RAI and accompanying documentation meet the standards set by Principle Long Term Care as well as Federal and State regulations.
- Engages in the training and development of interdisciplinary team members regarding the RAI process and quality measures.
- Completes initial, quarterly, annual, significant change, and special MDS assessments and care plans in accordance with regulatory guidelines.
- Ensures the prompt electronic submission of all MDS to the facility's designated scrubber.
- Facilitates the timely electronic submission of all MDS to iQIES.
- Accurately reports on resident conditions and services provided to enhance quality measure ratings and reimbursement from various payer sources.
- Understands and optimizes Case Mix Index (CMI).
- Responsible for educating skilled nursing staff on MDS processes and documentation standards.
- Collaborates with departments such as therapy and finance to ensure accurate and timely billing related to MDS.
- Conducts medical record reviews as required by regulatory bodies.
- Schedules and leads weekly care plan meetings.
- Participates in interdisciplinary team meetings as necessary.
- Develops appropriate care pathways and communicates essential information to residents, families, healthcare professionals, and third-party payers.
- Selects the correct reason for assessment and Assessment Reference Date (ARD).
- Reviews state validation reports and ensures necessary follow-up actions are taken.
- Schedules MDS assessments to maximize reimbursement opportunities.
- Completes accurate coding of the MDS based on medical record reviews and interviews with staff, residents, and families.
- Adheres to facility policies and procedures related to specific duties.
- Maintains MDS and care conference schedules.
- Completes assigned assessments to facilitate MDS completion.
- Reviews hospital records and documentation for relevant information regarding MDS.
- Engages with patients, families, and the public while working in an office environment.
- Performs additional duties as assigned.
- Ability to supervise, direct, and evaluate nursing staff in delivering resident care and fulfilling job responsibilities.
- Demonstrated effective communication, conflict resolution, and problem-solving skills.
- Knowledge of aging processes as they relate to healthcare, resident rights, and abuse regulations.
- Understanding of staffing needs and resource allocation for quality resident care.
- Demonstrates safe and correct techniques in using nursing and resident equipment.
- Effective oral and written communication skills.
- Professionalism and business acumen suitable for interactions with guests, clients, and healthcare professionals.
- Excellent time management skills and a sense of urgency in task completion.
- Ability to manage resources and supplies efficiently.
- Must possess a current RN license in New York State. A minimum of one year of experience in skilled nursing facilities, long-term care, rehabilitation nursing, or related healthcare settings, along with experience in MDS, RAI processes, and care planning. Current RAC-CT certification is required.
- Participation in seminars, workshops, and conferences as appropriate.
- Active involvement in in-service programs, Quality Assurance initiatives, and mandatory training.
- Attendance at various meetings and committee participation as required.
- Prolonged periods of sitting at a desk and using a computer.
- Ability to lift up to 15 lbs as needed.
- Occasional assignments to different units or shifts based on organizational needs.
- Compliance with health assessment and immunization requirements.
