Job Summary

The Case Manager I (CM I) plays a vital role in the interdisciplinary team, ensuring the highest quality outcomes during a patient's rehabilitation experience.

As a key member, the CM I coordinates and advocates for the patient from admission to post-discharge, managing information to effectively oversee healthcare delivery and facilitate interdisciplinary plan of care decisions.

The CM I oversees the effective coordination of services, managing issues in key areas such as admission and discharge, team conference and interdisciplinary plan of care communication, patient and family education, payor relations, and total fiscal management.

Additionally, the CM I performs ongoing utilization review, acts as a liaison to the payor, and ensures cost-effective treatment is provided by the team.

Key Responsibilities
