We are seeking an accomplished Nurse Practitioner In-Home Assessment professional to join our team. As a key member of our care coordination team, you will play a vital role in ensuring the highest quality of care for our patients.

**Key Responsibilities**

- Conduct thorough health risk assessments to identify patient needs

- Collaborate with patients and families to develop personalized care plans

- Provide ongoing support and education to patients and caregivers

**Requirements**

- Current RN and NP licensure in the state of practice

- Current CPR certification

- Enrolled in both Medicare & Medicaid
