About the Role

We are seeking a Clinical Care Coordinator to join our team at Beacon Health System. The successful candidate will be responsible for providing high-quality patient care, collaborating with multidisciplinary teams, and ensuring seamless coordination of efforts within the healthcare setting.

Key Responsibilities:

  • Provide short-term counseling and crisis intervention services to patients and their families.
  • Conduct comprehensive psychosocial assessments to determine treatment and discharge planning needs.
  • Collaborate with medical, nursing, and multidisciplinary staff to ensure effective patient care.

Requirements:

  • Master's Degree in Social Work (MSW) from an accredited institution.
  • Licensure or eligibility for licensure in the State of Indiana as a licensed clinical social worker.
  • At least 2 years of professional experience performing essential hospital or healthcare social services functions.

Estimated Salary:$72,500 per year
