Patient Care Coordinator

United Community & Family Services, Inc. is seeking a compassionate and dedicated Patient Solution Specialist to join our team. As a Patient Solution Specialist, you will be the primary point of contact for patients, ensuring they receive excellent customer service and their needs are met timely.
Key Responsibilities:- Act as the primary contact for patients, families, and caregivers, providing them with necessary information and support.
- Respond to patient inquiries, concerns, and complaints in a friendly and professional manner, addressing their needs with empathy.
- Perform insurance verifications to ensure clients receive relevant information about their insurance benefits for their initial intake appointment for medical, dental, and behavioral health services.
- Collaborate with healthcare providers, clinical staff, and other departments to ensure seamless delivery of care.
- Maintain accurate and up-to-date patient records, ensuring their privacy is protected in compliance with HIPAA regulations.
- Educate patients on their healthcare options, including insurance coverage, financial assistance, and other resources available to them.
- Provide initial and ongoing telephonic and in-person support to clients/patients seeking assessment for social determinants of health (SDOH) such as basic needs; legal, medical, housing, transportation, insurance, by offering and completing a comprehensive needs assessment.

Requirements:- High school diploma or equivalent; associates in healthcare or related field preferred.
- Three to five years related experience and/or training; or equivalent combination of education and experience.
- Excellent interpersonal, communication, and customer service skills.
- Strong organizational and time management skills, with the ability to multitask and prioritize effectively.
- Proficiency in Microsoft Office and experience with electronic medical records systems.
- Knowledge of medical terminology and insurance regulations.
- Ability to work independently and as part of a team, with a positive and proactive attitude.
- Flexibility to work occasional weekends or evenings to meet patient needs.
