About the Role

Sunrise Senior Living is seeking a compassionate and dedicated Care Manager to join our team. As a Care Manager, you will play a vital role in providing exceptional care and services to our residents in our assisted living and reminiscence neighborhoods.

Key Responsibilities
  • Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Develop and implement Individualized Service Plans (ISPs) to meet the unique needs and preferences of each resident.
  • Participate in the development of ISP monthly updates and ensure that residents receive the highest level of care and services.
  • Collaborate with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
  • Respond to the dining needs of residents and guests, maintaining Sunrise hospitality and service standards.
  • Encourage and assist residents in life skills and other life-enriching activities, as indicated on their ISP and demographic profile.
Requirements
  • Dedication to serving seniors with excellent customer service skills.
  • Positive attitude and flexibility to perform various duties in service to residents.
  • High School diploma or GED accepted, with certifications and licenses required in states where applicable.
  • Previous experience working with seniors preferred.
  • Ability to make choices, decisions, and act in the resident's best interest.
  • Competent in organizational and time management skills.
  • Demonstrate good judgment, problem-solving, and decision-making skills.
