About the Role

The Stroke Nurse Navigator plays a vital role within the multidisciplinary team, bringing specialized nursing knowledge in Stroke care. This position serves as a clinical resource, advocate, and educator, dedicated to assisting patients in navigating the complexities of the healthcare system and ensuring timely access to treatment and follow-up care.

Key Responsibilities

The Stroke Nurse Navigator is responsible for:

  • Coordinating comprehensive patient care from diagnosis through survivorship for individuals with stroke or TIA diagnoses.
  • Conducting concurrent and retrospective reviews of stroke patient records to gather data on compliance with stroke core measures and hospital policies.
  • Providing relevant data to regulatory agencies as required.

Qualifications Required

To be considered for this position, candidates must possess:
  1. A minimum of three years of nursing experience, with at least two years in Stroke Patient Care preferred.
  2. Current RN licensure in the District of Columbia or eligibility.
  3. A nursing baccalaureate degree from an accredited institution.
  4. Stroke Certified Registered Nurse (SCRN) credential from the American Board of Neuroscience Nursing, or the ability to obtain certification within 12 months of hire.
  5. NIH Stroke Scale and CPR Certification.

Compensation

The salary range for this position is a minimum of $43.57 per hour to a maximum of $67.53 per hour, with compensation determined based on experience and equity for similar roles.
