We are seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator. This role will involve working collaboratively with physicians, staff, and other healthcare professionals to provide care coordination across the healthcare continuum for a high-risk population.
Key Responsibilities:
- Coordinate self-management support and disease registry activities for the identified population.
- Use data to help manage the health of the identified population.
- Work closely with the Practice Transformation Quality team.
- Provide exceptional interpersonal customer service, including an outgoing positive attitude, warm friendly demeanor, and utmost care and respect for guests, patients, families, customers, and colleagues.
Requirements:
- Education: RN Required; BSN preferred.
- Experience: Minimum three (3) years' experience in a hospital, home care, or managed care setting.
- Required Licensure: Must have a current Washington State RN license.
- Other: Bilingual and/or English-Spanish speaking abilities preferred. Ability to work with a culturally diverse population.
