Position Summary
The primary role of the facility-based Advanced Practice Provider (APP) is to deliver provider-level direct patient care services within the post-acute care environment.

Key Responsibilities
  • Perform clinical interventions and episodic direct care visits, along with necessary re-evaluations of care.
  • Provide ongoing assessment and management of chronic, co-existing health conditions as clinically appropriate.
  • Assess and treat residents experiencing changes in their health status.
  • Review and manage medications, orders, and diagnostic tests, responding to clinical abnormalities for all patients under their care, as permitted by collaborating physicians.
  • Collaborate with physicians to ensure comprehensive medical management.
  • Conduct Annual Wellness Visits for all long-term nursing facility residents and may perform Annual History and Physicals as requested.
  • Assist with facility-wide health issues, where appropriate collaborations exist.
  • Coordinate care with other healthcare professionals, including therapists, specialists, and social workers.
  • Maintain communication with families and participate in family conferences when feasible.
  • Undertake additional duties as assigned.

Required Qualifications
  • Valid nurse practitioner or physician assistant license in the state of practice and current board certification in the respective field.
  • Master’s Degree in Nursing or Physician Assistant Studies.
  • Preferred strong leadership, interpersonal, and problem-solving abilities.
  • Excellent written and verbal communication skills are essential.
  • Must demonstrate punctuality, organizational skills, and the ability to work effectively within a team.
