About the Job:

We are seeking a skilled Nurse Practitioner to join our team and help us redefine health care. As a key member of our team, you will conduct comprehensive health risk assessments for Medicare, Medicaid, and Commercial Marketplace members/patients of primary care physicians, medical groups, and health plans we serve.

\

The ideal candidate will have 2 years of hands-on experience as a Nurse Practitioner and be registered with Medicare and Medicaid. They will have a current, unrestricted license in the state of assignment and must have license/certification: APRN, Certified Nurse Practitioner.

Responsibilities:

  • Conduct comprehensive health risk assessments\
  • Identify and refer members/patients for routine, urgent, and emergent case management\
  • Make recommendations for member's follow-up with their primary care physician\
  • Effectively communicate with elderly and chronically ill members\
\
