Job Description

We are seeking a skilled and compassionate RN Case Manager to join our team at CenterWell Home Health. As a RN Case Manager, you will play a vital role in delivering high-quality patient care and coordinating services that meet the unique needs of our patients.

The successful candidate will have a strong background in nursing, with a minimum of one year of experience in acute or home health settings. A diploma, associate, or bachelor's degree in nursing is required. The ideal candidate will possess excellent communication and organizational skills, with the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' medical, functional, and social needs
  • Develop and implement individualized care plans that address patients' goals and priorities
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination
  • Provide ongoing education and support to patients and families to promote optimal health outcomes
  • Manage and evaluate the effectiveness of care plans, making adjustments as needed
