Hemodialysis Care Coordinator

We are seeking a detail-oriented Hemodialysis Care Coordinator to join our team. As a critical member of our dialysis care team, you will be responsible for coordinating patient care and ensuring seamless transitions between treatments.
Key Responsibilities:
- Coordinate patient care and develop individualized treatment plans.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Ensure accurate laboratory collections and processing procedures.
- Identify expected outcomes and update nursing assessments and plans of care.
Requirements:
- Graduate of an accredited School of Nursing.
- Current appropriate state licensure.
- Minimum of 9 months experience as a Registered Nurse.
- Experience in chronic/acute hemodialysis (preferred).
