About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Duke University Hospital in Durham, North Carolina. As a Clinical Pharmacist, you will provide clinical pharmacy services, including order verification, clinical monitoring, and support of clinical pharmacy and departmental programs.

Responsibilities
  • Maintain outstanding skills as a clinical pharmacy practitioner and consistently demonstrate the value of pharmacy services.
  • Provide clinical pharmacy services to surgery and medicine services, including cardiology, pulmonary, critical care, internal medicine, general surgery, solid organ transplant, orthopedics/plastics, trauma, and vascular surgery.
  • Provide discharge medication reconciliation and contribute to ongoing hospital and department transitions of care initiatives.
  • Educate medical, nursing, and pharmacy staff about the management, prescribing, and monitoring of therapies used in patient care areas of responsibility.
  • Serve as a resource to the Department of Pharmacy for areas of expertise.
  • Participate in departmental and hospital committees and initiatives related to practice area.
  • Contribute to student and resident education.
  • Provide educational lectures and/or participate in research.
  • Work with peers to achieve division and departmental goals.
  • Take responsibility for personal and professional growth.
Requirements
  • Doctor of Pharmacy Degree or BS Pharmacy Degree.
  • Experience: With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required.
  • PGY1 or Pharmacy Practice Residency preferred.
  • 2+ years of practice experience preferred.
  • Licensed Pharmacist or eligible to practice pharmacy in North Carolina.
