Position Summary: We are seeking a highly motivated and skilled clinical pharmacist to join our team of inpatient oncology pharmacists in delivering exceptional care to adult medical oncology patients. The successful candidate will provide clinical pharmacy services, including rounding, limited order verification, and clinical monitoring, as well as partnering with physicians and other providers to promote evidence-based, safe, effective drug therapy.
Responsibilities:
- Partner with physicians and other providers to provide clinical pharmacy services and to promote evidence-based, safe, effective drug therapy.
- Serve as the medication expert to nurses, physicians, and other healthcare staff by providing drug information and ensuring the optimum use of medications.
- Provide education to patients and healthcare staff.
- Provide comprehensive supportive care and other medication therapy management.
- Partner with ambulatory colleagues to optimize patient transitions of care.
- Maintain outstanding skills as a clinical pharmacy practitioner and consistently demonstrate the value of pharmacy services.
- Educate medical, nursing, and pharmacy staff about the management, prescribing, and monitoring of therapies used in patient care areas of responsibility.
- Serve as a resource to the Department of Pharmacy for areas of expertise.
- Provide educational lectures and/or participate in research.
- Work with peers to achieve division and departmental goals.
- Take responsibility for personal and professional growth.
- Maintain familiarity with all departmental programs and work as an integral part of the pharmacy team.
Minimum Qualifications:
- Doctor of Pharmacy Degree or BS Pharmacy Degree.
- With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required.
- With Doctor of Pharmacy degree, no experience is required.
- 2+ years of practice experience preferred.
- PGY1 or Pharmacy Practice Residency preferred.
- PGY2 in oncology, Internal Medicine or Palliative Care or equivalent experience preferred.
Degrees, Licensures, Certifications:
- Licensed Pharmacist or eligible to practice pharmacy in North Carolina.
- Hold or Achieve Board Certification in Oncology Pharmacy or Pharmacotherapy within one year of hire.
