Clinical Pharmacist Endocrinology Specialist

About the Role:
The Clinical Pharmacist Endocrinology Specialist will provide clinical pharmacy services to patients served by Duke Endocrinology. This role will involve providing services for endocrinology disease states, including developing therapy plans, facilitating medication access, and providing patient education.
Key Responsibilities:
- Participate in clinic and care management activities to provide clinical pharmacy services and oversight for safe use of medication.
- Prescribe and monitor medications within the scope of practice designed collaboratively with physician colleagues.
- Screen for drug interactions, therapeutic duplications, omissions, etc.
- Monitor patients for therapeutic and/or adverse response to medication.
- Coordinate patient care and ensure appropriate follow-up, including laboratory monitoring, with other outpatient or inpatient teams.
- Ensure continuity of care and access with all medications.
- Facilitate prescription fulfillment through Duke specialty and retail pharmacies, as appropriate.
Requirements:
- Doctor of Pharmacy Degree OR BS Pharmacy Degree.
- Experience in pharmacy practice, with a BS Pharmacy degree requiring two years of experience, or acceptable board certification, or completion of an accredited residency program.
- PGY1 or Pharmacy Practice Residency preferred; with 1+ years of clinical practice experience or experience in managing endocrinology patient populations preferred.
