Clinical Pharmacist Position Overview

Key Responsibilities:
Deliver evidence-based pharmacotherapy guidance and patient education.


Identify, prevent, and resolve medication-related issues by evaluating patient medication regimens, ensuring appropriate medication use, monitoring dosages, suggesting therapy adjustments when necessary, and assessing cost implications for patients.

Provide counseling to patients regarding their medication plans.

Order, interpret, and monitor laboratory tests.

Manage adverse drug reactions and prevent drug-induced complications.

Develop and oversee medication care strategies, communicating these plans effectively with patients and the interdisciplinary team.

Document identified medication therapy issues and clinical outcomes.


Offer consultations to clinic physicians and other healthcare providers to enhance patient care.

Conduct retrospective medication utilization reviews.

Collaborate with third-party payers or government programs to optimize care costs and improve patient outcomes.


Evaluate medication usage patterns for potential issues or opportunities for optimization, ensuring compliance with medication fills, identifying therapeutic duplications, and making informed recommendations based on findings.

Handle billing for pharmacy services.


Enhance patient medication management and education through initiatives such as Annual Wellness Visits, Chronic Care Management, Co-Consults, and Transitions of Care.

Utilize Outcomes MTM for Comprehensive Medication Reviews (CMR) and follow up on Targeted Intervention Programs (TIPs).

Document office visits, consultations, and clinical outcomes of recommendations.

Quality Improvement Leadership:
Lead and direct quality improvement initiatives that enhance patient care and operational efficiency.

Analyze quality improvement metrics and assist in developing strategies to improve outcomes for Medicaid, Medicare, and Commercial plans.


Collect, record, and interpret data to implement strategies in collaboration with administrative and clinical leaders.

Track and present the results of improvement efforts and ongoing clinical process measures to management.

Qualifications:
Graduation from an accredited College of Pharmacy with a Doctor of Pharmacy (PharmD) degree. Completion of an accredited PGY1 Residency is required. PGY2 Ambulatory Care Residency is preferred. Must possess or be eligible for a pharmacy practice license in the State of Oregon. Board Certification is required within one year of hire, with BCACP preferred. Demonstrated experience as an ambulatory care pharmacist is preferred.

Proven expertise in assessing patient medications, educating patients, documenting care plans, and consultations within an electronic medical record system.

Experience in precepting pharmacy students and residents. Strong communication skills, ability to work collaboratively, effective time management, problem-solving capabilities, and the ability to work independently with minimal supervision are essential.

Work Schedule:
Four 8-hour days dedicated to direct patient care, with one day allocated for administrative tasks from home.

Compensation:
$145,000 annually ($87/hour) plus potential bonuses.
