Job Title: Community Resource Coordinator - Field

Description:

We are seeking a qualified Community Resource Coordinator - Field to join our team at HealthCare Support. This role will involve working closely with community-based organizations to provide health care support services to members. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:

  • Program Development: The successful candidate will have opportunities to develop and implement programs to promote health and wellness in the community.
  • Outcome Reporting: The candidate will be responsible for reporting outcomes and successes of outreach efforts, as well as identifying barriers to service.
  • Clinical Documentation System: The Community Resource Coordinator - Field will utilize a clinical documentation system to track and document interactions with members and providers.
  • Member/Provider Outreach: The candidate will conduct telephonic and face-to-face outreach to members and providers to promote health care services.
  • Health Plan Liaison: The successful candidate will serve as a liaison between the health plan and community-based organizations.
  • Health Risk Assessments: The Community Resource Coordinator - Field will conduct health risk assessments and screenings to identify potential health risks.
  • Social Barriers: The candidate will address social barriers to health care access, including closed-loop referrals.

Qualifications:

  • Location: The position is located in Orange County.
  • Community Health Worker Certification: A Community Health Worker Certification is required for Florida.
  • Working Knowledge of Social Determinants: The candidate should have a working knowledge of social determinants of health barriers, including safe housing, violence, education, job opportunities, income, nutritious food, physical activity opportunities, polluted air and water, language and literacy skills.
  • Working Knowledge of Geographic Area: The successful candidate should have a working knowledge of the geographic area and community-based organizations.
  • Motivational Interviewing: The candidate should have a working knowledge of motivational interviewing and care plan development.
  • Microsoft Office Suite: The successful candidate should have a general understanding of Microsoft office suite, including Word, Outlook, and Excel.
  • High School Diploma or GED: A high school diploma or equivalent is required.
  • Driver's License: The candidate should have a clear and active driver's license, reliable transportation, and car insurance.

Preferred Qualifications:

  • Bilingual in Spanish: Bilingual in Spanish is preferred.
