Job Title: Patient Navigator

Blue Ridge Health is seeking a Patient Navigator to join our Clinical Team. As a Patient Navigator, you will play a vital role in facilitating improved healthcare access and quality for our patients through advocacy and care coordination.

Responsibilities:
  • Screen patients for eligibility and assist with completion of necessary applications and paperwork for various programs, including sliding fee scale/discount fee program, subsidized health insurance programs, charity care programs, and medication assistance programs.
  • Conduct enrollments into programs when possible and provide referrals to partner agencies as needed.
  • Maintain certification as a Certified Application Counselor for the organization and complete all required training to maintain certification.
  • Provide Senior's Health Insurance Information Program (SHIIP) counseling to Medicare beneficiaries as requested.
  • Educate patients on health center advocacy and voter registration and provide related assistance.
Requirements:
  • Bachelor's degree in healthcare or social services-related field, preferred. Equivalent combination of education and experience is acceptable.
  • Familiarity with Medicaid, Medicare, Health Insurance Marketplace, private health insurance, SNAP, and other local, state, or national social safety net programs strongly preferred.
  • Ability to be certified as a Certified Application Counselor (CAC) and Senior's Health Insurance Information Program (SHIIP) counselor within one month of hire.
  • Valid driver's license and reliable transportation, required.
