About the Role

We are seeking a compassionate and skilled Nurse Practitioner to join our team at Abode Care Partners. As a key member of our interdisciplinary care team, you will provide high-quality medical care to seniors and individuals with complex needs in their homes, assisted living facilities, or independent living communities.

Responsibilities
  • Plan, direct, and evaluate patient medical needs, providing evidence-based care and collaborating with caregivers and other healthcare professionals.
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses, ordering and interpreting laboratory and diagnostic testing as needed.
  • Provide referrals to other healthcare practitioners when necessary and ensure patient medical records are kept up to date.
Qualifications
  • Active state Nurse Practitioners license and current Nurse Practitioner National Board Certification.
  • Active state DEA licensure and Master's degree in Nursing (MSN).
  • Five or more years of nursing experience, with at least two years of nurse practitioner experience preferred.
  • Strong interpersonal communication skills, excellent time management skills, and proficiency in medical instruments and equipment.
  • Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
  • Licensed driver with valid automobile insurance and ability to engage in moderate physical activity.
