Palliative Care Intake Coordinator

POSITION SUMMARY: The Intake Coordinator position with Three Oaks Palliative Care provides centralized intake and Community Liaison support to optimize patient care. The Intake Coordinator will complete the referral and admission process, provide effective communication, and collaborate with the Sales team.
Key Responsibilities:
- Coordinate referral information, receive required information from referral sources, and enter data into the Electronic Health Record (EHR) system.
- Verify insurance benefits and pre-authorization as required by specific insurance plans.
- Ensure all documents are received from third parties and internal documents are completed to process referrals and admissions.
- Manage patient scheduling, including admissions, rescheduling, confirming appointments, and map routing.
- Interact with Community Liaisons to ensure patients advance through the registration process and are scheduled to be seen promptly.
- Communicate timely and accurately with Community Liaisons to ensure coordinated and comprehensive care.
- Answer inbound calls from patients, referral sources, and payers.
- Respond promptly and professionally to emails and voicemails.
- Provide effective communication with patients/family, team members, and other healthcare professionals to educate on TOPC's services.
- Develop and maintain effective working relationships with Community Liaisons, Case Managers, Managed Care Organizations, and other pertinent payers/referral sources.
- Prepare reports and participate in care coordination meetings and case conferences where applicable.
- Maintain and permanently protect the confidentiality of patient information, enforcing patient rights regarding privacy, personal property, and grievances.
- Comply with Medicare, Medicaid, and HIPAA regulations and requirements at all times.
Qualifications:
- High School Diploma or equivalent; Bachelor's Degree preferred.
- 1-2 years related experience.
- Prior insurance verification experience.
- EMR/EHR experience preferred.
- Creative, resourceful, results-oriented, and collaborative nature.
- Detail oriented with a sense of urgency and timeliness for reporting.
- Proficient in MS Office including Word, Excel, and PowerPoint.
- Strong communication skills (written and verbal) and desire to work in cross-functional groups.
- Ability to communicate with various external and internal clients in a professional and courteous manner.
- Ability to multi-task in a fast-paced environment driven by measurable results.
- Self-starter with the ability to organize work for maximum efficiency.
Working Conditions:

The employee frequently is required to reach with hands and arms. The employee is required to stand, walk, and stoop.
