About the Position:

During a visit that can last up to one hour, Matrix providers assess and evaluate a member's current health status, medical history, medication compliance, social circumstances, and other risk factors.

This process offers unparalleled insight into a member's overall health and well-being that is often challenging to capture during standard office visits.

The Matrix Comprehensive Health Assessment aims to enhance the quality of care and allows us to potentially address multiple care gaps in a single encounter.

Responsibilities:
Matrix Provider – What to Expect:
  • Conduct Adult/Geriatric Assessments, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and thorough documentation in accordance with Matrix guidelines and protocols.
  • Collaborate effectively with physicians, case managers, social workers, family members, key caregivers, and relevant ancillary medical personnel as necessary.
  • Engage with Primary Care Physicians (PCP) on patient education and provide follow-up care.
  • Deliver services across various settings, including Home Visits, Skilled Facility Visits, and Telehealth Consultations.

Compensation:
$110.00 per completed assessment.

Assessment Locations:
Home, Skilled Nursing Facilities, Telehealth, and other community environments.

Flexible Scheduling Benefits:
PRN/Part Time providers are non-benefited except for 401(k) and limited Voluntary Benefits. Mileage Reimbursement plan available.

Qualifications:
Must Haves:
  • Master's Degree required OR equivalent experience and successful completion of NP licensure.
  • Master's Degree required OR equivalent experience and successful completion of PA licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain it.
  • Board certified by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • A minimum of 1 year of experience as a Nurse Practitioner/Physician Assistant.
  • Ability to travel as needed.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • A positive and outgoing personality, and the ability to work as part of a team.

Travel Requirements:
Must possess a valid state driver’s license and be able to drive a vehicle, with proof of adequate automobile insurance coverage for the state of residence. Travel is required to meet patients in their homes.
