Job Summary

The Supportive Housing Case Worker provides ongoing case management services to individuals and families in recovery from substance use disorder. This role involves establishing positive professional relationships, providing and coordinating supports to maintain housing stability, and linking clients to community resources that promote economic independence.

Key Responsibilities
  • Establish and maintain positive professional relationships with clients
  • Provide case management services, including intakes and assessments
  • Assess and facilitate access to mainstream benefits (SSI, SSDI, SNAP, TANF)
  • Link clients to community resources that promote economic independence
  • Teach necessary skills, such as transportation, budgeting, and life skills
  • Provide advocacy and teach self-advocacy in accessing needed services
  • Work with clients to create and achieve Smart Goals with an Individual Service Plan
  • Complete all required reports and tracking tools for clients, funding sources, and the agency
Requirements
  • Bachelor's degree or 3 years' equivalent experience required
  • Ability to work effectively with diverse populations
  • Good typing/computer skills
  • Lived experience preferred
  • Must have valid driver's license
  • Bi-Lingual / Bi-Cultural applicants encouraged to apply
