Job Summary

St. Elizabeth Physicians is seeking a skilled RN Care Coordinator to join our team. As a key member of our healthcare team, you will work collaboratively with providers, interdisciplinary staff, and clinical associates to support patients with chronic conditions and/or complex needs.

Key Responsibilities
  • Facilitate effective communication and coordination of services to address patient needs.
  • Provide education and guidance to patients related to current health concerns.
  • Work collaboratively with providers to develop and implement care plans.
  • Coordinate with interdisciplinary staff to ensure comprehensive patient care.
  • Monitor patient progress and adjust care plans as needed.
Requirements
  • Current State Allied Health License required.
  • Current Driver's License in good standing and reliable and insured transportation.
  • Degree in nursing or other related clinical field.
  • Care Management Certification preferred.
  • Three to four years medical field experience.
  • Demonstrated knowledge of anatomy and physiology, pharmacology, etc.
  • Ambulatory and/or care management experience preferred.
Preferred Skills and Knowledge
  • Ability to manage and prioritize multiple tasks.
  • Knowledge of electronic Health Records (EPIC).
  • Knowledge of Excel, Word, Outlook, and PowerPoint, and the ability to learn other computer skills as needed.
  • Good organizational skills.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal skills.
  • Ability to affect change.
  • Ability to perform critical analysis.
  • Self-directed.
  • Work well telephonically as well as face to face.
  • Can work autonomously.
  • Be familiar with motivational interviewing with patients.

  • Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events.
  • Must respond and follow through to requests from customers promptly.
  • Must work carefully and precisely with attention to detail.
  • Must utilize resources wisely.
  • Performs duties willingly and with initiative. Shares necessary information so co-workers can do the same.
  • Cooperates with other departments and work groups.
Additional Requirements
  • Previous Quality Assurance experience preferred.
Licenses and Certifications
  • Current state Allied Health license.
