Innovative Renal Care is seeking a skilled Nurse Care Specialist to join our team. As a Nurse Care Specialist, you will provide high-quality patient care to individuals suffering from End Stage Renal Disease (ESRD). You will work closely with patients and their families to improve their quality of life and create meaningful relationships.
Responsibilities:
- Provide patient care to individuals with ESRD, including administering medications, monitoring vital signs, and performing dialysis treatments.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Conduct patient education and counseling on disease management, treatment options, and self-care techniques.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
- Associates Degree in Nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (BSN); three year diploma from an accredited diploma program may be substituted for nursing degree.
- Registered Nurse License in the state of practice.
- Current BLS/CPR certification required.
- Meets all applicable state/licensing requirements.
- Previous dialysis, ICU, CCU, ER, or Med Surg nursing experience is preferred but not required.
