Patient Navigator
The Patient Navigator plays a crucial role in managing relationships and fostering referral growth within a designated market. This position involves engaging with physician offices, networking, and participating in professional healthcare organizations, as well as supporting community initiatives such as health fairs, screenings, and other community-driven activities aimed at enhancing awareness of PAM Health's programs and services. Additional responsibilities may be assigned as needed. This is an exempt role that reports directly to the Senior Liaison, with oversight from the Director of Business Development.
KEY RESPONSIBILITIES
Marketing Initiatives
Identifies and targets potential referral sources for PAM Health within the assigned area. Establishes and nurtures strong relationships with referral sources by continuously assessing and addressing their rehabilitation needs. Conducts in-person meetings to promote PAM Health programs to designated referral sources, achieving weekly marketing objectives. Communication must be clear, concise, and accurate. Maintains a comprehensive understanding of all inpatient and outpatient programs, including their features and benefits across the organization. Achieves set goals for weekly outreach, joint marketing efforts, professional tours, and educational presentations.Reporting and Documentation
Completes and submits weekly call reports promptly. Time and expense reports, along with receipts, must be submitted by the second working day of each month. Keeps an up-to-date and thorough record of all contacts within the assigned territory using the sales database. Maintains customer profiles (e.g., Physician, Case Manager, Employer) in the database and updates them as necessary. Contributes to ongoing market research by collecting relevant information and sharing it with management. Provides insights to management regarding identified needs or activities that could enhance the organization's marketing efforts. Assists in developing new programs and improving services through continuous assessment of market stakeholder needs. Identifies new business and partnership opportunities and communicates these to management.Managed Care Engagement
Recognizes opportunities for establishing contracts with managed care organizations and provider networks. Coordinates meetings with managed care organizations and PAM Health Case Managers or the Director of Managed Care as appropriate.Customer Service Excellence
Upholds the highest standards of customer service through courtesy, compassion, and effective communication.
Advocates for the Mission and Vision of PAM Health within the workplace and the community. Respects the dignity and confidentiality of all individuals by adhering to applicable policies and procedures.QUALIFICATIONS
Education and Training: A valid license in nursing, therapy, or social work, or substantial healthcare experience in marketing. Proficiency in computer use and a desire to enhance skills with PAM Health's information systems.
Experience: A minimum of two years of experience in healthcare sales or marketing is required. Previous experience engaging with physicians is preferred.
Knowledge, Skills, and Abilities:
Strong interpersonal skills. Excellent written and verbal communication abilities. Experience in presenting information to individuals or organizing educational sessions for PAM Health staff. Familiarity with Microsoft Word, Excel, and PowerPoint for creating presentations on PAM Health services. Effective decision-making capabilities. Ability to thrive in an unstructured environment.
