POSITION OVERVIEW

UofL Physicians is in search of an advanced practice clinician to collaborate closely with the CVICU (Cardiovascular Intensive Care Unit) specialists and adult cardiothoracic surgical teams at our facility. The clinician will deliver comprehensive care to patients undergoing cardiothoracic procedures in both step-down and intensive care units. This role requires flexibility for day and night shifts, with specific requirements to be discussed during the interview process.

ROLE RESPONSIBILITIES

The practice involves daily patient rounds and surgical progress documentation for all cardiothoracic patients. Responsibilities include both inpatient and outpatient management, which entails patient evaluation, treatment plan development, implementation, reassessment, care coordination, follow-up, and thorough discharge summary completion.

The clinician will also supervise and mentor registered nurses in the management of cardiothoracic patients.

In partnership with physicians and a multidisciplinary healthcare team, the clinician will participate in cardiovascular service line committees, educate staff on protocols, and provide patient education and disease management.

KEY FUNCTIONS

The patient population includes, but is not limited to, individuals with acute decompensated heart failure, complex post-operative cardiac surgical cases, acute myocardial infarction, complicated endocarditis, aortic dissections, and patients requiring percutaneous temporary ventricular assist devices and LVADs.

  • Conduct comprehensive histories and physical examinations, consultations, and daily surgical progress notes for adult cardiothoracic patients in the CVICU and step-down units.
  • Assess and manage chest tubes, epicardial wires, and central venous lines.
  • Manage anti-arrhythmic medications, anticoagulation, and titration of inotropic medications.
  • Provide outpatient post-operative follow-up care.
  • Evaluate the physical and psychosocial status of patients through interviews, health histories, physical examinations, and diagnostic studies.
  • Demonstrate the ability to manage multiple priorities in a fast-paced environment.
  • Recognize deviations from normal in physical assessments and collaborate with physicians to formulate treatment plans.
  • Prescribe medications, blood tests, and therapeutic measures as needed.
  • Request consultations from other healthcare professionals to ensure quality patient care.
  • Interpret and evaluate findings from studies and tests.
  • Communicate relevant patient care information to collaborating physicians.
  • Evaluate care quality and recommend improvements.
  • Initiate actions to implement therapeutic plans aligned with patient needs.
  • Work with a multidisciplinary critical care team to provide patient-centered care.
  • Engage in discussions regarding ethical issues and patient wishes in treatment decisions.
  • Communicate respectfully with patients, families, and healthcare team members regarding treatment decisions and prognosis.
  • Collaborate effectively with others and share clinical expertise with healthcare professionals.
  • Document and report care accurately and in a timely manner.
  • Promote evidence-based practice and participate in clinical research.
  • Discharge hospitalized patients under the direction of attending physicians and complete discharge summaries.
  • Maintain professional certification in the specialty area and participate in educational conferences.
  • Work a varied schedule, including weekend and holiday coverage.
  • Perform other duties as assigned.

QUALIFICATIONS

Education and Certification: APRN: Must be a graduate of an accredited program with a Master of Science in Nursing. Board Certification is required. Acute Care certification focus is preferred but not mandatory.

PA: Must be a graduate of a physician assistant program accredited by the Accreditation Review Commission on Education for Physician Assistants or its equivalent and have passed the certifying examination administered by the National Commission on Certification of Physician Assistants.

  • Required Licensure: APRN and RN in the state of Kentucky.
  • Physician Assistant in the state of Kentucky.
  • Other certifications: ACLS/BLS certified (can be obtained upon employment). DEA certification to be obtained upon employment.
