Family Service Association is seeking a dedicated Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing care coordination to children with mental health needs and their families.

**Key Responsibilities**

* Provide care coordination to children with mental health needs and their families
* Facilitate the development of the Care Planning Team
* Utilize multiple tools, including strength-based assessments, to organize and guide the development of an individual care plan and a risk management/safety plan
* Work with youth and their families in their home and community

**Requirements**

* Master's Degree in Social Work, Counseling, or Psychology, or related field, with a license or license eligible
* OR Bachelor's Degree in a human services field with one year of relevant experience working with families or youth
