Position Overview: This role is a vital part of the leadership team and a key element of the Juniper Village Wellness Staff. The Healthcare Liaison Specialist enhances the process and communication flow among primary care providers, ancillary services, therapies, community teams, residents, and families.

Key Responsibilities:

Primary Care Coordination:

  • Evaluates resident schedules and compiles a list of residents for daily appointments, ensuring an efficient clinic workflow; measures vital signs including weight, blood pressure, pulse, and temperature, and documents findings; prepares necessary paperwork/orders for practitioners and provides assistance as required.

Resident Support and Coaching:

  • Engages one-on-one with residents and/or families to set self-management objectives; organizes appointments and conducts follow-up visits as necessary; reminds residents of upcoming appointments and coordinates transportation; collaborates with the Wellness Team to communicate any follow-up needs for residents.

Administrative Duties:

  • Onboards new residents and maintains existing residents' electronic records, ensuring all necessary reports, releases, and lab results are filed prior to appointments; conducts monthly audits of charts for accuracy; updates insurance information as needed; discusses insurance payment options with residents, families, and providers.
  • Schedules external appointments as required, manages clinic supply inventory; triages and processes messages from residents and front office staff for physicians; performs duties as a medication technician or licensed practical nurse as needed.

Qualifications:

  • High school diploma or equivalent.
  • Successful completion of an accredited medication administration course or licensed LPN.
  • Completion of CPR and First Aid training (or ability to complete within 30 days of hire).
  • Familiarity with the healthcare sector, medical terminology, and the physiology and psychology of older adults.
