Position Summary

Fountain House is a community-based organization dedicated to supporting individuals with serious mental illness (SMI) through a collaborative and inspiring movement. We are seeking a Mental Health Social Practitioner to join our team and contribute to our mission of fostering growth, collaboration, and positive impact.

The Mental Health Social Practitioner will work in our brand new Clubhouse in Harlem, supporting members in their recovery journey through social practice and community engagement. This role requires a strong understanding of mental health principles, excellent communication skills, and the ability to work effectively in a dynamic team environment.

Key Responsibilities

  • Support and develop a therapeutic community based on Social Practice and the International Standards for Clubhouse Programs.
  • Provide training support for transitional employment opportunities and maintain strong relationships with employers.
  • Assist members with entitlements and benefits, including food stamps, SSI/SSDI, Medicaid/Medicare, and ticket to Work/ACE/Access VR.
  • Advocate for members' well-being, safety, and livelihood with supports around clinical, legal, housing, appointments, medications, and independent living opportunities.

Requirements

  • 1-3+ years' experience working with individuals with serious mental illness and/or hands-on experience working in a community setting.
  • 1-2 years coaching and counseling experience preferred.
  • Ability to effectively manage and handle member concerns and behaviors.
