Counseling and Case Management Specialist

This role involves working with clients to provide individual and group counseling, case management services, and program activities to support them in achieving their goals and maintaining sustainable housing.
Job Summary:
- Provide counseling and case management services to clients.
- Collaborate with Clinical Supervisor as needed.
- Develop and implement treatment plans for clients.
Responsibilities:
- Conduct individual and group counseling sessions.
- Assess client needs and develop treatment plans.
- Provide referrals to community-based resources as needed.
- Maintain accurate and up-to-date client files and records.
- Participate in team meetings and contribute to discussions on client progress.
Requirements:
- Minimum of 1 year experience in Human Services or related field.
- Certification or license in substance abuse counseling (CADAC or LADAC) or at minimum enrolled in a certificate program.
- Ability to work independently and collaboratively, with strong written and verbal communication skills.
- Valid driver's license, reliable transportation, and meeting insurance standards.
