About the Role

We are seeking a highly skilled Service Coordinator to join our team at Presbyterian Villages of Michigan. As a Service Coordinator, you will play a vital role in ensuring that our residents maintain a good quality of life and age in place.

Job Summary

  • Coordinate programs and services to meet the physical, mental, psychological, and social needs of residents.
  • Assess and identify resident and family needs, and provide referrals to outside resources as needed.
  • Network with community organizations to seek affordable and new services for residents.
  • Initiate and complete psychosocial assessments, interviewing patients, families, and others to evaluate current conditions and identify potential adjustments.
  • Provide general case management and referral services, maintaining complete and updated resident files.
  • Coordinate services for optimal living, including aging in place and wellness philosophies.
  • Complete Care Plans and Quarterly Monitoring with residents needing referrals, and follow up on all services at and after implementation.
  • Submit Semi-Annual Performance Reports to HUD to maintain funding for the Service Coordination program.

Responsibilities

  • Engage all residents to identify areas of need and make necessary referrals.
  • Organize and promote resident involvement in on-site and off-site programs.
  • Assist in the coordination of home care services to enable independent living and aging in place.
  • Develop a Resource Directory listing service providers.
  • Educate residents and staff on various matters, including service availability, application procedures, resident rights, health and safety issues, and aging-related topics.

Requirements

  • Minimum degree in social work, psychology, or gerontology preferred.
  • Two or more years of experience in social service delivery with elderly and/or family populations.
  • Demonstrated working knowledge of supportive services and other resources in the area served by the project.
  • Demonstrated ability to advocate, organize, problem-solve, and provide results for residents served.

Skills and Qualifications

  • Good communication, comprehension, and interpersonal skills.
  • Working knowledge of Windows, Microsoft Office Suite, and resident software data systems (AASC online, Care Guide, RealPage, or similar).

Compensation and Benefits

We offer a competitive salary range of $45,000 - $60,000 per year, depending on experience. Additionally, employees have access to early wage earnings through PayActiv. We also offer a comprehensive benefits package, including opportunities for professional growth and development.
