Key Responsibilities- Leadership and Management
- Establish and implement daily priorities, tasks, and communication plans for the nursing staff.
- Assist nursing staff, residents, families, and other team members in identifying resident needs and providing support.
- Resident Care and Services
- Conduct daily independent rounds with other team members to observe and assess resident care, needs, and outcomes.
- Develop and implement resident care plans in collaboration with the interdisciplinary care team.
- Ensure accurate and timely completion of Minimum Data Set (MDS) assessments.
- Staff Development and Training
- Develop and implement training protocols for short-term rehabilitation programs.
- Ensure all staff members complete and understand training protocols.
- Quality Improvement and Compliance
- Monitor and investigate CMS Quality Indicators triggered for unit residents.
- Formulate and implement plans to improve resident outcomes and decrease quality indicators.
- Administrative and Reporting
- Prepare and submit reports and audits to the Director of Nursing as required.
- Assure compliance with facility policies, state, and federal mandates.
- Graduate of a registered professional nursing school.
- Current NYS Registered Professional Nurse License.
- BSN and Master's in Nursing Administration preferred.
- Two (2) years of Geriatric Nursing or Long Term Care Supervisory experience.
