Nurse Case Manager

Job Summary: Geisinger Health System is seeking a skilled Registered Nurse - Case Manager to join our team. In this role, you will be responsible for assessing, planning, implementing, and evaluating the care of patients with complex medical conditions.
- Assess Patient Needs: Conduct comprehensive assessments of patients to identify their healthcare needs and develop individualized care plans.
- Coordinate Care: Work closely with healthcare teams to coordinate care and ensure seamless transitions between providers.
- Monitor Progress: Continuously evaluate patient progress and make adjustments to care plans as needed.
- Apply Benefits and Utilization Management: Apply benefits and utilization management principles to ensure optimal patient outcomes.
Requirements: To be successful in this role, you will need a Graduate from Specialty Training Program-Nursing and a Minimum of 3 years-Related work experience. You should also be a Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania.
