Job Description: Mental Health Care Coordinator

We are looking for a dedicated and empathetic Mental Health Care Coordinator to join our team at Bay City Residence in Perth Amboy, NJ. This individual will be responsible for coordinating mental health services for consumers with psychiatric disabilities and co-occurring medical issues.

The successful candidate will work closely with the program manager, RN, DDD case manager, and other treatment team members to ensure comprehensive delivery of services. They will provide direct care services, including transportation, support, teaching, crisis intervention, and case management.

Key Responsibilities:

  1. Coordinate mental health services for consumers, including transportation, support, teaching, crisis intervention, and case management.
  2. Work closely with the program manager, RN, DDD case manager, and other treatment team members to ensure seamless delivery of services.
  3. Develop and implement individual habilitation plans in collaboration with the treatment team.
  4. Document observations, information, and services provided to the consumer resident in progress notes.

Requirements:

  • A bachelor's degree in mental health, health, or a related field, or equivalent experience.
  • A valid driver's license with a clean driving record for at least 24 months.

Salary: $19.37 per hour (hourly rate may vary based on experience).
