Job Overview

We are seeking a dedicated Mental Health Specialist - Care Coordinator to join our team at Metrodoc Urgent Care. As a key member of our healthcare team, you will provide high-quality care and support to patients with mental health needs.

Key Responsibilities
  • Assess Patient Needs: Conduct thorough assessments of patient needs, identifying areas of concern and developing targeted interventions to address them.
  • Develop Treatment Plans: Work with patients to create personalized treatment plans that take into account their medical history, mental health status, and personal goals.
  • Provide Individualized Care: Deliver compassionate, person-centered care to patients, focusing on their unique needs and circumstances.
  • Patient Education: Educate patients and their families on mental health conditions, treatment options, and self-care strategies.
  • Collaboration: Work closely with multidisciplinary teams, including psychiatrists, therapists, and social workers, to ensure comprehensive care and support for patients.
Requirements
  • Master's Degree: Possess a Master's degree in nursing from an accredited program.
  • Certification: Hold certification as a family nurse practitioner.
  • Fellowship: Have completed a fellowship in psychiatric-mental health nurse practitioner.
  • Experience: Bring 2+ years of experience in providing psychiatric care to your role.
  • Skills: Demonstrate strong clinical skills and judgment, excellent communication and interpersonal skills, and the ability to work independently and as part of a team.
