The University of Rochester Medical Center is seeking a visionary leader to serve as the Division Director for the Division of Transitional Care Medicine. This esteemed position offers a unique opportunity to shape the future of healthcare and make a lasting impact on the lives of patients and families.
Key Responsibilities- Provide strategic leadership and direction to the division, ensuring alignment with departmental and institutional goals.
- Foster a culture of excellence, innovation, and collaboration among faculty, staff, and students.
- Develop and implement effective clinical, educational, and research programs that address the complex needs of patients with chronic conditions.
- Build and maintain strong relationships with stakeholders, including patients, families, healthcare providers, and community partners.
The Division of Transitional Care Medicine is a dynamic and interdisciplinary team dedicated to providing comprehensive care to youth and adults with chronic conditions. Our team is committed to delivering high-quality, patient-centered care that addresses the unique needs of each individual.
- We provide outpatient clinical care at the Complex Care Center, an innovative interdisciplinary primary care setting.
- Inpatient care settings include adult inpatient medical care at Highland Hospital, intensive behavioral therapy at Golisano Children's Hospital, and inpatient consultation at Strong Memorial Hospital.
- We offer a range of educational activities, including fellowship, residency, and student programs, as well as robust grant activities and evolving research infrastructure.
- MD, DO, or equivalent degree.
- Board certification in Internal Medicine (BC/BE in Pediatrics preferred).
- Immediate eligibility for licensure in New York.
- Strong leadership, academic, and clinical experience.
- Experience working with the targeted transitional patient population (preferred).
- Broad academic experience in research and education, with a preference for candidates with an established track record of peer-reviewed research and extramural funding.
