Job Summary

Alta California Regional Center (ACRC) is seeking a highly skilled Service Coordinator to join our team in the Supported Living Services Unit. As a key member of our team, you will provide case management and advocacy services to individuals with developmental disabilities, ensuring they receive the necessary support and resources to thrive.

Key Responsibilities
  • Assist clients and their families in acquiring and maintaining assessed supports and services, including transportation, vocational training, day programming, education, mental health services, medical care, and independent living training.
  • Serve as an advocate for clients with community agencies, ensuring their needs are met and their rights are protected.
  • Consult with and assist vendors with certifications, applications, and referrals, ensuring seamless service delivery.
  • Participate in the development of Individual Program Plans for clients, monitoring and revising them as necessary to ensure optimal outcomes.
  • Complete all required forms, documentation, and reports in accordance with regulations and ACRC policies and procedures.
Requirements
  • Bachelor's or Master's degree in Social Work or a related field.
  • Excellent written, verbal, and interpersonal communication skills.
  • Strong time management and organization skills.
  • Valid California driver's license and automobile liability insurance.
  • One year of professional experience in social work or a related field, preferably in developmental disabilities.
