INPATIENT CASE MANAGER - SOCIAL WORKER

Job Summary:

The Inpatient Social Worker plays a vital role in promoting patient and family wellness, improving care outcomes, and accessing appropriate hospital and community resources for patients with complex health needs.

Key Responsibilities:

  • Collaborate with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention.
  • Develop and implement individualized care plans to address patients' physical, emotional, and social needs.
  • Facilitate communication between patients, families, and healthcare providers to ensure seamless care transitions.
  • Identify and address social determinants of health that impact patients' well-being.
  • Provide education and support to patients and families on health-related topics and community resources.

Requirements:

  • Master's degree in Social Work (MSW) required.
  • 2 years of hospital social work or community social services experience preferred.
  • LMSW required; LCSW/LCSW-C preferred.
  • 3-5 years of experience in a healthcare setting.
  • MD Social Work License per level of education.
