Inpatient Case Manager RN

At LifeBridge Health, we are seeking a skilled Inpatient Case Manager RN to join our team. As a key member of our care management team, you will play a vital role in promoting patient wellness, improving care outcomes, and ensuring efficient utilization of health services.
Key Responsibilities:- Assess patient needs and develop individualized care plans to ensure seamless transitions from inpatient to outpatient care.
- Collaborate with the clinical team and medical providers to initiate and implement discharge plans.
- Stay up-to-date on changing organizational, regulatory, and payer requirements to ensure compliance and optimal patient outcomes.
- Provide education and support to patients and families to promote self-management and empowerment.
- BSN or ADN degree required; BSN preferred.
- 3-5 years of experience in inpatient case management or discharge planning.
- Current Maryland RN license; intent to obtain MD licensure if out-of-state.
- Case Management certification preferred.
