Inpatient Case Manager RN

At LifeBridge Health, we are seeking a skilled Inpatient Case Manager RN to join our team. As a key member of our care management team, you will play a vital role in promoting patient wellness, improving care outcomes, and ensuring efficient utilization of health services.
Key Responsibilities:- Assess patient needs and develop individualized care plans to ensure seamless transitions from inpatient to outpatient care.
- Collaborate with the clinical team and medical providers to initiate discharge plans and ensure timely and effective care coordination.
- Stay up-to-date on changing organizational, regulatory, and payer requirements to ensure compliance and optimal care delivery.
- Provide education and support to patients and families to promote self-management and empowerment.
- Basic professional knowledge and working knowledge of theory and practice within a specialized field.
- BSN preferred; ADN required.
- 3-5 years related experience; prior experience with inpatient case management/discharge planning preferred.
- Maryland Registered Nurse License.
- Intent to achieve MD licensure if out of state.
- Case Mgmt cert preferred 3-5 yrs of hire for role.
