Inpatient Case Manager

At LifeBridge Health, we are seeking a skilled Inpatient Case Manager to join our team. As a key member of our care management team, you will work collaboratively with our clinical team and medical providers to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention for our high-risk inpatient population.
Key Responsibilities:
- Provide patient and family advocacy to ensure their needs are met and their voices are heard.
- Coordinate discharge planning to ensure a smooth transition from hospital to home or other care settings.
- Provide psychosocial intervention to promote patient and family wellness and improve care outcomes.
- Collaborate with our clinical team and medical providers to develop and implement individualized care plans.
- Identify and address social determinants of health that impact patient outcomes.
Requirements:
- Master's degree in Social Work (MSW) or related field.
- 2+ years of experience in hospital social work or community social services agencies.
- LMSW required; LCSW/LCSW-C preferred.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
