Job Summary:

The Hospice Social Worker is a vital member of our interdisciplinary team, responsible for providing comprehensive social services to patients and their families. This role requires a strong understanding of the hospice philosophy and the ability to promote positive working relationships within the company and with external stakeholders.

Key Responsibilities:

  • Explain hospice services and Medicare benefits to patients and families, obtaining informed consent and election of benefits documents as needed.
  • Submit accurate and timely documentation and paperwork to facilities, adhering to company policy.
  • Participate in the development and implementation of patient care plans, collaborating with the interdisciplinary team to ensure continuity and quality of care.
  • Provide psycho-social support and counseling services to patients and families, identifying and addressing concerns related to financial means, insurance, living arrangements, caregiver stressors, and community resource needs.
  • Facilitate placement in long-term care facilities, monitoring adjustment and supporting advance planning, including living wills, POA documents, funeral arrangements, and memorial services.

Requirements:

  • Minimum of a Bachelor's degree in Social Work, Psychology, Sociology, or a related field, with one year of social work experience in a healthcare setting.
  • Licensure required, with supervisory oversight by an MSW for hospice patient care planning and counseling needs.

Skills and Abilities:

  • Strong written and verbal communication skills, with the ability to articulate and embrace the hospice philosophy.
  • Ability to understand, read, write, and speak English.
  • State-specific requirements, including licensure to practice social work in Kansas.

Work Environment:

The Hospice Social Worker will work in a healthcare setting, with regular standing, walking, and manual dexterity required. Adaptations can be made to accommodate team members with disabilities. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
