Key Responsibilities- Assess and develop individualized care strategies for our members, focusing on the most appropriate and least restrictive level of care.
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
- Manage the care plan throughout the continuum of care, serving as a single point of contact for our members and their families.
- Communicate effectively with all stakeholders to ensure quality coordinated care and services are provided promptly to our members.
- Advocate for patients and families as needed, ensuring their needs and choices are fully represented and supported by the healthcare team.
- Associate's degree or higher.
- 4+ years of experience working in a community health setting in a healthcare role.
- Intermediate Microsoft Office experience, including MS Word, Excel, and Outlook.
- Experience in long-term care, home health, hospice, public health, or assisted living.
- Ability to travel in assigned regions to visit Medicaid members in their homes and/or other settings.
- Licensed social worker or behavioral health/clinical degree.
- Experience working with the brain injury population.
- 2+ years of case management experience.
- 1+ years of experience working with Medicaid Waiver services.
- Experience with arranging community resources.
- Background in managing populations with complex medical or behavioral needs.
- Field-based work experience.
- Experience with electronic charting.
