Hospice Social Worker

The Hospice Social Worker is a vital member of our interdisciplinary team, responsible for providing comprehensive psychosocial support to patients and their families. This role requires a strong understanding of the Compassus values and philosophy, as well as the ability to model these values in daily interactions.
Key Responsibilities
- Develop and implement individualized care plans that address the unique needs of each patient and family.
- Provide education and support to patients and families regarding hospice services, Medicare benefits, and other relevant topics.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
- Assess and address the psychosocial needs of patients and families, including financial, emotional, and spiritual concerns.
- Facilitate advance care planning, including living wills, power of attorney, and funeral arrangements.
- Participate in care planning meetings and provide input on patient care.
- Document all interactions and interventions in a timely and accurate manner.
- Master's degree in Social Work (MSW) or related field.
- Minimum of one year of experience in a healthcare setting, preferably in hospice care.
- Licensure as a social worker, as required by state regulations.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Excellent organizational and time management skills.
- Ability to maintain confidentiality and handle sensitive information.
This role requires the ability to work in a fast-paced, dynamic environment, with frequent interactions with patients, families, and healthcare professionals. The Hospice Social Worker must be able to adapt to changing circumstances and prioritize tasks effectively. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
